Our integrated team of home health and hospice experts are trained to carefully consider all of the challenges that their patients and their loved ones may face, including their medical, psychological, spiritual, financial and social needs. Our integrated team refuses to let patients “fall through the cracks.” Our new, innovative Frontier Integrated Care Solutions (FICS) program provides support and timely resources for all of our patients at all stages.
